This presentation outlines the results of academic research that was carried out on Process Safety at the University of Sheffield in 2018 by Mohamed.

The research examines 100 major process incidents that occurred in different industries; of which 10% of these incidents occurred in the steel industry.

The research aims to find out answers to the following questions:

What is the correlation between different stages of the asset lifecycle and process safety incidents?
What is the impact of the process safety incidents on people?
What are the immediate and root causes of Process safety Incidents?
How can we prevent process safety incidents?
